d)Electronic Submission of your Application

Due to COVID-19 Concerns, DC Health is doing online renewals and applications 1) You can make an appointment for telemedicine here ( scroll to Find A DC Doctor):https://nationalholistic.com/become-a-patient 2) After this appointment you will receive an email with the referral number and link to the online application from DC Health 3) DC Health will email a payment link  3 business days after the application in 2) is submitted 4) DC Health will email a temporary card with your image and new expiration date or physically mail you a card.
